PLEASE NOTE THAT WE ARE NOT IN A POSITION TO ACCEPT NEW PATIENTS AT PRESENT. While normally we welcome new patients, we are currently at the maximum number of patients we can continue to provide a safe service for, particularly during the ongoing pandemic.

If you would like to register with us:

• Please complete our New Patient Registration Form before your first consultation.

• We request a contact mobile phone number.

• We provide a Transfer of Medical Records Request Form, to enable the release of your previous files to us.

• If you have a medical card, then you must bring your medical card with you.

• Medical card patients must complete a Change of Doctor Form, as well as the New Patient Registration Form.

• The clinic is fully computerised with all medical records kept safely backed-up off-site on secure servers within Ireland.
